Development of OA Following ACLR: Unexpected Finding in Medial Vs. Lateral Meniscus Pathology

Recent studies have described increased risk of OA following ACL reconstruction (ACLR). Systematic reviews suggested that meniscal pathology, which occurs in up to 60% patients with injury, increases this risk. The literature reports a 50% incidence medial meniscectomy, two times the compared lateral meniscectomy. purpose study was determine influence pathology on ACLR. Between 1999-2015, all who underwent primary autograft or allograft BPTB transtibial ACLR by single surgeon (KDP) were identified. Revision ACLR, multi-ligamentous reconstructions, and < 2-year imaging follow-up excluded. Meniscus recorded. defined at as Kellgren-Lawrence (KL) grade III-IV plain radiographs. Independent samples t-tests assessed group differences demographic variables chi-square tests rates (p<0.05). Eighty-five (48 males, 37 females; age 38.5±10.9 years) included an average 8.69±4.88 (range 2-20 years). prevalence meniscus tear time injury 40% (34/85) tears (43/85) (p=0.166). Medial compartment seen 7% (6/85) patients. Prevalence greater patient (8.8%; 3/34) those without (5.9%; 3/51); however, not significant (p=0.604). Of 3 patients, 2 had partial meniscectomy 1 repair. Lateral 15.3% (13/85). (25.6%; 11/43) (4.8%; 2/42) (p=0.008). Patients 4.47 [95%CI: 1.1 17.4} more likely develop OA. Eleven 35 (31%) developed 0% (0/7) repair (p=0.084). While multiple surgeons rehabilitation protocols shown development is we found significantly higher these 8.69 years follow-up. historically, thought be protective against OA, did find superior cohort reconstruction.
